The NEEDS Outreach Transition Page
The NEEDS Outreach Transition Page is designed for teachers, school personnel, parents and community members who work with kindergarten through twelfth grade students, and most importantly for the students themselves. This website is a tool to be used by these stakeholders to assist students in learning the skills necessary for success after high school.
Some students will be able to do many of the activities independently. Other students will need assistance and adaptation of materials. (See contact information below.)
This site is a kindergarten through twelfth grade curriculum with printer-friendly materials accessible via the Internet focused on the needs of exceptional students. We would appreciate any feedback or examples of adaptations that you have made to meet the individual needs of your students.
Students can access this site any time during their education, and begin building a portfolio that will assist them in transition. There are forms that can be downloaded in either PDF or Microsoft Word format that students can do independently, or with the assistance of a teacher and/or parent. The Microsoft Word documents can be typed in, saved, and printed. They can also be revised in the future.

The NEEDS Outreach Transition Page Mission:
The NEEDS Outreach Transition Page provides a sequential framework of activities and resources across the curriculum for career awareness, career exploration, personal documentation, work and volunteer experience documentation, and career preparation. The framework of this page helps students, parents and teachers to build a fundamental connection between school and careers. Students begin with career awareness associated with their family and school experiences and progress to exploring occupations, acquiring work skills, and compiling portfolios to prepare for life after high school. Collaboration among students, parents, teachers, and the community is crucial to the success of this mission.
In the Future
This site will not only be useful during kindergarten through twelfth grade, but many of the skills and resources collected in the portfolio from high school should be saved forever. Each time people change jobs, homes, marital status, have new experiences, training or learn new skills, they should update their resume and personal information. Please revisit this page in the future, and utilize any of the resources available.
This website is authored by Janet Terry and Joyce Brody, teachers in the Deaf and Hard of Hearing Department at Madison High School in the San Diego Unified School District, San Diego, California. Both Joyce and Janet have been teaching Deaf and Hard of Hearing students for over 25 years.
